Appointment Types

Rechecks

Rechecks are follow-up appointments for a previous diagnosis. Some of the most common rechecks are scheduled for ear infections, mononucleosis (mono), pneumonia, asthma, burn care, cellulitis and concussions. The time frame for the recheck is determined by the specific provider.


Medication Checks

This type of visit is reserved for children taking specific medications. The most common medications are those for the treatment of attention deficit hyperactivity disorder (ADHD). These visits are made to ensure that the medication is working properly, monitor for adverse reactions, maintain effective dosing and monitor growth and behavior. These appointments are generally every 3 months.


Pre-Operative Visit

This visit is sometimes required by surgeons prior to a child undergoing surgery. These appointments are usually requested 1 week prior to the operation.


Sports Physical

This visit is required for a child to participate in competitive athletics. This type of visit is scheduled if a child has had a physical within the past calendar year, but requires another physical before 365 days have pasted. This type of visit is not usually covered by insurance.


Weight Checks

These appointments are made to check a patient's weight. This visit is often scheduled within 2 to 5 days after hospital discharge for breastfeeding newborns. This visit maybe necessary to monitor the growth and nutrition of children who maybe overweight or underweight for age and height.


Immunization Only Visit

This visit is designed for the sole purpose of vaccinating. A patient will see a nurse, not a doctor at this visit. If you would like to be seen by a doctor, please reschedule a regular sick appointment.


Referrals

Most managed care insurance plans require referrals if you are seeking care from anyone other than your primary care provider. Please check with your insurance company to see if a referral is needed. To request a referral for your child, leave a message on extension 105 with the following information:

  • Child's Name (spell the last name)
  • One phone number where you can be reached
  • Name of the insurance company
  • Name of the specialist you are being referred to
  • Time & date of your appointment & NPI number for the specialist
  • Reason for you visit


A minimum of 48 hours is needed to complete referral requests. We do not back date referrals.


Labs, Radiology, Specialists & Hospital Services

Please contact your insurance company to verify the status of network participation by specialists, facilities, labs or in-hospital services for your specific plan. We do the very best we can to assure you use high quality facilities and see physicians who deliver the best quality care. We cannot assume responsibility for network participation as is changes on a daily basis.


Transfer of Medical Records

If you are joining our practice please contact your former physician for their policy and procedures regarding transferring your child's medical records. If you wish to transfer your records to another practice, $15 per child and a signed letter of authorization is required. You may hand deliver, mail or fax this request to our office. When the request is completed you may pick up your records, or we are glad to mail the records to you or the new physician's office. We do not fax medical records. Additional fees apply to obtain medical records from our archives facility.
